We welcome you to our Diabetes Information Event
Forest Health Group PPG (Patient Participation Group) and Binfield Surgery CCG would like to warmly invite you to our forthcoming educational evening on Diabetes at Easthampstead Baptist Church on Wednesday November 12th at 7pm. We invite patients with Type 2 Diabetes, at risk of Type 2 Diabetes, carers of patients with Type 2 Diabetes, care home staff and others to this event.
During the evening a variety of health professionals will talk to you about aspects of Type 2 Diabetes, prevention, diagnosis and much more. And on the whole it should be fun and interactive. You shall have plenty of time to ask questions if you wish to.
Please be aware, that you must register your attendance. Only registered patients can be admitted. The form will ask you also for an email address and a telephone number in case of last minute changes. There are about 200 spaces. We expect this evening to be quite popular and would recommend early registration. Once the spaces are full the web registration tool will be disabled, but you will still be able to express your interest for future events. If we get enough expressions of interest we will stage another event. Registration finishes on Tuesday 11/11/2014 at 1pm at the latest or when the event is full, whichever comes first.
Who: All patients registered with a practice in Bracknell & Ascot or any person working within the NHS or the Local Authority or associated services.
