Register Now For Our Diabetes Follow Up Event
During the Diabetes Evening in November many of you requested the opportunity for a follow up event.
Forest Health Group PPG (Patient Participation Group) and Binfield Surgery PPG would like to warmly invite you to our forthcoming follow up event on Diabetes at Easthampstead Baptist Church on Wednesday March 4th 2015 at 6:45 for 7pm.
This event is solely for patients, who attended the first event on Diabetes on Nov 18th 2014.
Please be aware, that you must register your attendance on the bottom of this page. Only registered patients can be admitted. The form will ask you also for an email address and a telephone number in case of last minute changes. There are about 200 spaces. We expect this evening to be quite popular and would recommend early registration. Once the spaces are full the web registration tool will be disabled.
